Description:
Bridging the Gap Family Services provides mentoring services to individuals ages 8 through 20. Individuals must be pre-approved for services through Family Assessment and Planning Teams (FAPT) and Wraparound Teams because the program is funded through the CSA (Comprehensive Services Act). Mentoring Services are a lower level of care for those young people that DO NOT meet the Intensive In-Home criteria.
